SUBJECT: Distribution of CAIB Report

I understand that there were some problems with the delivery of the Columbia Accident Investigation Board (CAIB) report prior to Safety and Mission Success week. These difficulties were due in part to our having received fewer reports than we needed. Headquarters has since sent us more copies of the report. We will try to get them to anyone who didn’t receive the report the first time, and would like one. We will also look into reasons for the delivery problems, in order to correct them in the future. We look forward to receiving your comments from SMS week.
